Medical Insurance

Patients are legally responsible for their medical expense regardless of whether they have any medical insurance or not. Payment for services is expected by most medical practices at the time of service unless arrangements are made prior to the treatment. Patients must pay their copays or deductibles due at the time services are rendered. We provide a service to our patients by filing the insurance claim with their insurance providers. Some insurance benefit plans may require a referral from the primary care physician prior to office evaluation or follow-up and it is the responsibility of the patient to obtain this referral.

Current insurance information is imperative for correct billing. Let us know who provides your primary and secondary insurance. Please present your insurance card to the receptionist when you arrive for your appointment and inform us of any changes to your insurance or billing information.

The following is a partial list of the insurance carriers we currently have contracts with. If you do not find your plan on our list, please contact your insurance provider before you receive your medical care. You are always ultimately responsible for the expense of your medical care and we do not want you to be saddled with large medical bills because of any misunderstandings.
